Information and Resource Referrals
Our Information and Assistance Specialists will provide education and advocacy services for health care, living arrangements, employment and training, mental and emotional support services, and many other community resources.
Benefit Enrollment and Assistance
Our Benefits Specialists will provide you with accurate and unbiased information about public and private benefits. We assist with filing applications for Social Security Income (SSI) and Social Security Disability Income (SSDI), SNAP (food stamps), Medicaid and Medicare. We can help you cut through some of the “red tape”.
Our Options Counselors help you create a care plan which meets your needs and preferences including: where you would like to live, what type of care you may need in the future, who you may want to involve in your planning, and other considerations. We do this by collecting information from you, assessing your eligibility for various programs, and working with our community partners.
Our staff can provide residents with emergency assistance for utilities, rent, minor home modifications, transportation for medical appointments, and other emergencies.
